Are you sure she wants the cover to go over the cast? Maybe just knit a bit of ribbing and then the toe of a sock. It would slip over the exposed toes and kind of under the edge of the cast.
A toe-up pattern would be great for this..you could judge how "deep" to make it and then add just a few rows of ribbing to help hold it in palce.

toe cover

hi there. my friend took a nasty spill and now she has a cast with toes sticking out. she is requesting a....toe cover


i am a great patter follower but not a designer. i have knit two pair of socks but that was eons ago. any suggestions about how to get this accomplished? obviously, it needs to be big to get over her cast and stay put.

ooo. just had a thought. maybe its a hat for your foot...with a bit more ribbing?

or, maybe its a tube sewn at one end? nah.
